In boxes 1-5 list five things that help you prepare best for competition to help calm you in the lead up to the event. Some examples are listen to music, meditate, breathing exercises, write down your basketball achievements, talk to my coach or team mates and let them know how I’m feeling, reading, stretching etc.
In ‘Important Reminders’ write a simple affirmation statement to yourself eg “I got this,” “I believe in myself,” “I deserve to be successful.”
